Immunization and Registration
Registration for Pre-kindergarten, Kindergarten and First grade and all initial entries to Dade County Schools.

Requirements:

Child must be 4 years old by September 1st, for entering Prekindergarten.

Child must be 5 years old by September 1st for entering Kindergarten.

Child must be 6 years old by September 1st for entering First grade.

1. Must have original birth certificate.

2. Social Security number required.

3. Must Have 680 form, listing immunizations including Two Measles, Hepatitis B Vaccine Series and Varicella (Chickenpox) vaccinations or documentation for disease.

4. Must have proof that the child has received a physical examination within the last 12 month (including proof of tuberculin skin test and results and appropriate follow-up)

5. Must have proof of address, light bill, a lease or rental agreement on apt. or home.

6. If student comes from another school outside Dade County, we must have recent report card.

Matricula de Prekindergarten, Kindergarten y primer grado para entrar por primera vez en las escuelas del Condado de Dade.

Requisitos:

Nino/a que cumpla 4 años antes o Septiembre 1ro, para entrar al Prekindergarten.

Nino/a que cumpla 5 años antes o.Septiembre Iro para entrar al Kindergarten.

Nino/a que cumpla 6 años antes o Septiembre Iro para entrar al primer grado.

1. Necesita certificado de nacimiento original.
2. El numero del Seguro Social.
3. Necesita la forma 680 con todas las fechas de las vacunas incluyendo Dos Sarampion, vacunas contra la Hepatitis y las vacunas de varicela (la china) o documentaci6n del doctor.
4. Necesita prueba del examen fisico del medico con el resultado de la Tuberculosis reciente (no mas de 12 meses).
5. Necesita prueba de la direccion (recibo de la luz o contrato del apt. o de la casa).
6. Si el nino/a atendio otra escuela fuera de el Condado Dade. necesitamos su reporte de calificaciones mas reciente.

Attendance

Attendance at Palm Lakes Elementary is MANDATORY. If your child needs to go to the doctor for a check-up or for any medical reason, please try to arrange doctor's appointments after the school day is over. If this is not possible, please have your child bring a note the following day to school. Your child's teacher will only allow 5 notes written by you during the school year in order to consider an absence EXCUSED. After the 5 th parent note the school requires you to bring in a note from the doctor. If the notes are not from the doctor they will be marked UNEXCUSED. We currently work with the Truancy Intervention Program in the State Attorney's Office to make sure we have excellent attendance. After 10 UNEXCUSED absences, you will receive a letter from the State Attorney's Office arranging a meeting to discuss your child's truancy. Please don't let it get to this point and remember how important it is that your child be in school everyday by 8:20 a.m.

The assistant principal and I also monitor tardies during the school year. If your child has been late to school 10 or more times, you will receive a letter from the principal as a warning to get to school on time. If the tardies continue, you will receive a call from me to come in for a conference to discuss your tardy situation and help you improve it. At this time your child will also get a SCAM which will remain in his permanent record. Beyond this conference, if your child is tardy, your child's name will be referred to our school social worker. Again, please work with us this year and lets get your child to school on time everyday.

Uniform Policy

Our uniform consists of navy blue pants (no jeans), jumper, knee length shorts or skirt and white or light blue buttoned shirt or polo shirt (no T-shirts). All shirts/polos should have a patch on the left hand side.

On Fridays, students may wear the Palm Lakes Elementary T-shirt instead of the school uniform.

Nuestro uniforme consiste de pantelones (no jeans), jompi, short (largo por la rodilla). o saya (falda) azul prusia y la blusa de butones o un pulover polo blanco o azul clarol (no t-shirts). Todas las camisas o pulovers polo tienen que tener una insignia en el lado izquierdo.

Los viernes los alumnos pueden usar el "t-shirt" de la escuela en vez del uniforme.
